Universal Referral, Application & Follow-Up Systems
The Universal Systems Action Team is currently piloting a new Referral Follow Up process with cross sector organizations to make it easier for frontline workers to coordinate care for residents seeking to get connected to community resources.
This process includes a referral follow up form that can be used to improve the culture of communication among front line workers in Eaton county by improving information sharing specific to program referrals.
This team has also developed an Eaton County Referral Coordinator Contact list that can be viewed below.
