StreetHeart Neighborhood Program
StreetHeart is a resident-led neighborhood initiative rooted in the belief that strong connections between neighbors create healthier communities. Through neighborhood gatherings, block leader engagement, and grassroots strategies, StreetHeart fosters belonging, resilience, and local leadership.

HealthComm Program
HealthComm strengthens partnerships between healthcare providers, community health centers, and local organizations to improve access to care for Eaton County residents. By working collaboratively across sectors, HealthComm helps ensure that individuals and families can more easily connect to the services and supports they need.
A key focus of this work is increasing access to food and nutrition resources as essential components of health. Through coordinated outreach, shared strategies, and aligned efforts with healthcare partners, HealthComm helps address food insecurity, promote healthy eating, and connect residents to programs that support long-term wellness.
HumanServ Program
HumanServ focuses on building stronger collaboration across the human services sector. By bringing agencies and partners together, we work to reduce duplication, improve coordination, and create more effective systems of support for residents.

Agency Endowment Fund
The Agency Endowment Fund provides long-term, sustainable support for Eaton Community Health. Gifts to the endowment are invested, and a portion of the earnings is used each year to strengthen our programs and respond to emerging community needs.
