HumanServ

ECH teams work collaboratively throughout Eaton County, to create neighborhoods and communities where individuals and families thrive. We do this with an ‘upstream’ approach, serving as a catalyst for improving in the “social determinants of health” in our county.  All ECH programs are rooted in collective impact… a collaborative approach to improving our communities. We work with residents, local organizations, health care, and local government. Our HumanServ programs work with county Collaborative Councils to simplify access to resources and help better meet resident needs.

Working together to develop a centralized referral system for all organizations & coordinated process for information sharing to better coordinator care for Eaton county residents.

“Carry the message”
The Eaton County Human Services Collaborative Council came together in October 2023  to celebrate the incredible work of all three Action Teams who have moved into a new phase of Action Team strategy!
1. Universal Referral Follow Up

3. Easier to Access Services
The celebration theme centered around the role that each one of us has to “carry the message” of this work back to our Organizations to keep it moving forward.
We learned together how our collaborative Action Team work is shifting systems here in Eaton County to make it easier for residents to access services that meet basic needs.

Eaton County HSCC Action Teams: Celebration of 2022
We are so proud of the work of our Eaton County Human Services Collaborative Council (HSCC) Action Teams!
These action teams were formed in the late spring of 2022, and by the first week of December, we were celebrating the progress made toward meaningful systems change for Eaton County.
Our focus is improving community conditions so residents in our community may thrive.

 

 

We say THANK YOU to each person who contributed to this meaningful work in 2022, and can not wait to see what we can do together in 2023!
